Tertiary open tip rhinoplasty following to primary interventions in different centre through a closed and subsequently open approach. Significant residual asymmetry of the nasal bridge and tip improved through the tertiary intervention, reduction of the residual convexity of the nasal bridge and correction of the impression of down pointing nasal tip. Noted that the visibility of the nostrils from the front has not increased despite the shorter appearance of the nose.

Open tip primary ethnic rhinoplasty to produce a narrower overall appearance of the bridge and tip of the nose, and to correct the slightly convex contour of the bridge, associated with down pointing appearance of the tip on profile. The individual characteristics of the patient allowed for the correction to take place without using rib cartilage grafting. Visibility of the nostrils from the front is essentially unchanged despite the rotation of the tip.

Open-tip reduction rhinoplasty.Open-tip reduction rhinoplasty.Open-tip reduction rhinoplasty.

Open-tip reduction rhinoplasty. The patient's concerns were related to moderate over projection of the nasal tip, the presence of a bump on the bridge of the nose, a degree of deviation of the bridge of the nose on the front and wide nasal tip. The characteristics of the tip of the nose involved moderately wide cartilages and intermediate to thick nasal tip skin creating a parenthesis deformity. The correction involved reduction of the size and width of the nasal tip cartilages, reduction and resetting of the support of the bridge of the nose, shortening of the tip. Note minimal difference in the visibility of the nostrils from the front view despite nasal tip shortening.

Primary open-tip reduction rhinoplasty. Preoperative concerns related to nasal bridge deviation and width, slightly wide nasal tip excess nasal tip length and projection and slightly strong nasal bridge, giving a more masculine appearance to the nose. The intervention aimed to create a softer nasal contour, with narrowing of the tip and the bridge, significant reduction in the nasal length and reduction in projection of the nasal tip and bridge. Note that visibility of the nostrils from the front is essentially unchanged despite the significant shortening (up turning) of the tip.

Primary open-tip rhinoplasty for is reduction of the width of the tip of the nose and of the bridge. Also, reduction of the nasal tip projection and length (nasal tip shortening) as well as correction of the bump on the bridge of the nose. Note that the up turning effect of the tip of the nose on profile with a shorter and more feminine contour, is not associated with increased show of the nostrils from the front.

Open-tip reduction rhinoplasty, for correction of moderate over projection of the nasal tip, and correction of nasal bump. The intervention involved shortening of the tip of the nose for a balanced relationship between the length of the bridge and projection, reduction in the height of the bridge and refinement of the nasal tip cartilages.

Primary open-tip rhinoplasty for increased nasal tip support and definition using cartilage grafts from the nasal septum. The correction applied for the downward pointing nasal tip leads to the impression of a more upturned nose without increased nostril show from the front view. The bridge of the nose was reduced to correct the convexity, and in fractures of the nasal bones control the width of the bridge.

Ethnic rhinoplasty using rib cartilage graft. The third nasal reshaping intervention for this patient who had two previous treatments in different centres. The intervention involved enhancement of the bridge of the nose for definition, enhancement of the support of the nasal tip for definition and slight shortening, and slight reduction in the width and bulk of the nostrils.

Open tip secondary rhinoplasty, following a closed rhinoplasty in a different centre. The patient wanted correction of the irregularities of the bridge and tip of the nose from the front and profile, as well as a reduction in the projection of the nasal tip. The nose reshaping involved repeat in-fractures for a narrower contour of the top of the bridge, cartilage grafts to the bridge and tip of the nose, and reduction of the cartilages of the tip.

Primary open-tip rhinoplasty for correction of the nasal bump and reduction of the projection of the tip of the nose and increased definition of the tip from the front and profile. A reduction of the anterior nasal spine at the base of the nose reduces the appearance of upward pull on the upper lip. A small chin reduction procedure was associated with the nose reshaping for improved balance of the profile.

Primary open-tip rhinoplasty to straighten the bridge of the nose on profile, and reduce the projection of the tip of the nose. Reduction of the anterior nasal spine at the base of the nose reduces the tension on the upper lip for an improved profile balance. From the front, narrowing of the tip of the nose for increased definition.

Secondary open-tip rhinoplasty following first surgery in a different centre. Reduction rhinoplasty involved shortening of the nose, decrease in nasal tip projection, reduction of the height of the bridge, and slight enhancement at the radix (top of the bridge of the nose) for smoother transition between forehead and nose on profile. From the front, increased definition of the tip of the nose.

Secondary open-tip rhinoplasty following first surgery in a different centre. Correction of the nasal tip projection and length (shortening) together with slight lowering of the nostril rim, for reduced columella show on the side view and improved balance of the bridge and tip relationship on profile. Narrowing of the tip of the nose on the front view.

Open tip reduction rhinoplasty, thick nasal skin.Open tip reduction rhinoplasty, thick nasal skin.

Primary open tip reduction rhinoplasty for correction of over-projecting and down-pointing nasal tip (excess nasal length and projection), and for correction of the nasal bridge bump. Slight enhancement using cartilage graft at the top of the bridge creates a more balanced transition between the forehead and a bridge of the nose on profile. Narrowing of the tip of the nose from the front view. Because of this significant reduction in nasal tip projection, a compensatory alar base reduction (Nostril reduction) avoids flaring of the nostrils.

Primary open tip reduction rhinoplasty, with rotation (shortening) of the tip to attenuate the impression of dipping of the tip of the nose when smiling and reduced show of the coluemlla from side and 3/4 view. Small reduction in the projection of the tip to avoid imbalance with the relatively strong chin.

Reduction rhinoplasty, correction of the nasal bump.Reduction rhinoplasty, correction of the nasal bump.

Primary open tip reduction rhinoplasty; narrowing of the nasal tip and correction of the nasal bump. Nasal reshaping that involves hump reduction usually benefits from slight nasal shortening (slight upturning of the tip). Note however that the visibility of the nostrils from the front is essentially unchanged.

Tertiary open tip rhinoplasty following two previous closed rhinoplasty operations in a different centre. Primary problem related to polly-beak deformity, due to insufficient reduction of the cartilage of the nasal bridge; secondary problems related to retraction (peaking) of the nostril rim and excessive nasal shortening. Corrective surgery involving open-tip rhinoplasty with septal cartilage graft for lengthening, reduction of the cartilage excess above the tip.
by Lucian Ion (FRCS)

Tertiary rhinoplasty.Tertiary rhinoplasty.
Correction of nasal bump.Correction of nasal bump.

Primary reduction rhinoplasty through and open-tip approach. The patient's concerns were related to the nasal bump, excess nasal tip projection and excess nasal tip length with associated increased columella show and slight deviation of the bridge. The surgery involved reduction of the bone and cartilage framework of the nose, nasal tip shortening with no alteration in the nasolabial angle (parallel shortening), and narrowing of the cartilages of the tip. Note that from the front view the visibility of the nostrils is essentially unchanged despite the shortening.
by Lucian Ion (FRCS)

Primary open-tip reduction rhinoplasty, for correction of moderately wide nasal tip with slight deviation, moderate excess nasal projection and length. Reduction of the nasal tip width (boxy nasal tip) associated with correction of the deviation and slight shortening as well as lowering of the bridge of the nose. The view from below demonstrates the narrowing effect on the nasal tip, together with correction of the deviation of the columella and improved symmetry of the nostrils. Minimal visibility of the columella scar, and in the essentially no increase visibility of the nostrils from the front despite the increased nasolabial angle (nasal shortening).

Primary open tip rhinoplasty, correction of nasal bump.Primary open tip rhinoplasty, correction of nasal bump.

Primary open-tip rhinoplasty for wide (boxy) nasal tip, with relatively strong parenthesis deformity (shadows from the lower border of the alar cartilages), limited definition of the tip of the nose from the profile as well as small nasal bump on the bridge of the nose. The surgery involved reduction and the reshaping of the nasal tip cartilages, reduction of the cartilage and bone support of the bridge of the nose, slight narrowing of the bridge and shortening of the tip. Note essentially no change in the visibility of the nostrils from the front view despite the lift in the tip of the nose on profile.

Primary open-tip reduction rhinoplasty of for refinement relatively wide nasal tip with significant thickness of the nasal tip skin, and correction of the bump on the bridge of the nose. Significant narrowing of the tip of the nose was necessary, associated with lowering of the bridge, slight augmentation at the radix for smoother transition between forehead and bridge, nasal tip shortening for reduction of the lateral show of the columella and a more feminine profile.

Primary open-tip ethnic rib graft augmentation rhinoplasty. Reduction of the nasal tip and bridge width from the front view for increased definition, increased projection and definition of the tip and bridge on the profile, reduction of the fullness of the nostrils. The increased support for the nasal tip is provided by the rib cartilage graft which has a stronger texture then the nasal cartilages, and can create a stable contour even in the context of relatively thick nasal tip skin.
